Pathway plans must be reviewed regularly to make sure you are getting on ok when you leave care. While you are an ‘eligible child’ these reviews will be done as part of the review of your overall care plan.
When you are a ‘relevant child’, your plan must be reviewed not more than 28 days after you move to check that you have settled into your new home and that it meets your needs.
At that stage it will be decided whether you should have your next review after three or six months. It will depend how much support your personal adviser thinks you need.
The government thinks that if possible your local authority should review your plan within 28 days of any future move. If you are very unsettled or need extra support your plan should continue to be reviewed every 28 days until you are happier. If your personal adviser thinks that you need reviews more often they can arrange them.
Your views must be heard when arranging these reviews and you can say if you don’t want a particular person to be at the meeting. They might still have to be consulted though if they have something important to contribute.
