What will be in my care plan?

 

Lots more plans! But there’s a good reason for them – it’s so that people can make sure you are getting what you need in all areas of your life to have the best chance of being happy and achieving the things you need for a good life.

You can find out more about what will be in each of these five plans by clicking on the links above, but first here are some other very important things which must also be in your care plan:

 

  • It should say why you were taken into care and what children’s services did to try to keep you with your family
  • Information on your behaviour and your emotional development (this means how you generally feel about things and how your feelings come out, like for example whether you undertand and talk calmly about how you feel or whether you get very angry about things)Information about your identity, including your religion, your race and which languages you speak
  • Information on your relationships with family members and other people who are important to you
  • Information about how well you are able to look after yourself (self-care skills) and how you get on with people generally (social skills)
  • The name of your Independent Reviewing Officer (IRO)
  • How you, your parents and carers feel about the arrangements for your care – especially the placement plan and the pathway plan and any changes that might be made to it.
