Your placement

This is where you will live.

 

Your placement plan is part of your care plan and must be written before you go to live in a new place, or if that’s not possible then within five working days of moving there. It will say where you are going to live, who will look after you and how long it’s expected you will live there.

It should set out in detail why this placement is thought to be right for you and how it will meet your needs.

It will say clearly how day to day parenting tasks will be shared between your carer and the local authority and who will pay for the things you need. This is so you and your carer can be clear about who makes which decisions.

It will tell your carer about your needs and interests, your likes, dislikes and routines. It will say whether you have had any problems with previous carers and suggest ways for your carer to build a good relationship with you and avoid things that particularly upset you.

Your placement plan is also the part of your care plan which will say when and how you should see your parents, and also any brothers and sisters you have, if they aren’t living in the same placement with you.  If it has been decided that it’s better for you not to see certain members of your family it will say so in your placement plan and it must also say how any changes to this should be made.

It will say how often your social worker should visit you, and who you can contact between these visits if you need extra advice or support. It will say whether you have an independent visitor and what the arrangements are for seeing them.

The placement plan will also give the reasons why the placement might end.

It will also contain contact details for your IRO, independent visitor and social worker.
